

The Land Transportation Office (LTO) on Wednesday issued a show cause order (SCO) against the registered owner of a dump truck involved in a fatal road crash along Sumulong Highway in Barangay Dela Paz, Antipolo City, Rizal, on Tuesday, November 25.
In the SCO, LTO Chief Assistant Secretary Markus V. Lacanilao directed the agency’s Intelligence and Investigation Division (IID) to summon Ms. Cristina Alcazar de Castro, the registered owner of the dump truck, or her authorized representative.
According to the Spot Report from the Antipolo City Police, the dump truck allegedly lost its brakes, causing it to slam into the rear of another dump truck and pushed both trucks forward, leading them to collide with five vehicles.
Authorities confirmed that three drivers, including the dump truck driver, were declared dead on arrival (DOA). Four other individuals were reported injured and rushed to the nearest hospital.
A relative or representative of the deceased driver was also ordered to appear in an investigation scheduled on November 28, 2025, at the LTO Central Office in Quezon City.
Both parties are required to submit a verified or sworn explanation detailing why they should not be held administratively liable for the operation of a vehicle with defective or unauthorized accessories, devices, equipment, or parts.
Ms. de Castro has also been ordered to provide the Motor Vehicle Inspection Report (MVIR) to prove the roadworthiness of the dump truck involved in the accident.
Additionally, the family or representative of driver was instructed to surrender his driver’s license to the LTO for safekeeping until the case is fully resolved.
